PERSONAL CARE AND HOMEMAKING SERVICES
Wherever you call home, we will be there for you, anytime of the day or night. Two hour minimum.
PERSONAL CARE ASSISTANCE
Assistance with personal care such as bathing, dressing, grooming, skin care, meal preparation, bathroom assistance, incontinence care and transfers.
RESPITE CARE AND COMPANIONSHIP
Relief for caregivers in the home or in a healthcare setting. We will keep your loved one company so you can take a break.
HOMEMAKING SERVICES
Housekeeping tasks to maintain a safe and clean living environment. We can also run errands.
CONCIERGE HOME HEALTH AIDE CAREGIVER PROGRAM
Provides our most clinically experienced staff members who have specialized training in dementia care, rehabilitation and geriatric services.
PHILIPS LIFELINE MEDICAL ALERT SERVICE
Montefiore at Home will help you maintain your independence and feel safe in the comfort of your own home. We have partnered with Philips Lifeline to provide prompt caring 
assistance at the push of a button, 24 hours a day, 365 days a year.

TRANSPORTATION SERVICES
Our Private Care staff can provide transportation in conjunction with your personal care needs. Transportation is available for medical appointments and other outings as requested. We will accompany the client and use the clients’ car.
EVENT ESCORT
Accompaniment to special events such as holiday celebrations, family gathering, graduation party, bar mitzvah, bridal or baby showers, funeral or other events.
NURSING ASSESSMENT AND EVALUATION
Registered Nurses (RN) provide support and serve as the liaison with the client’s physician to coordinate healthcare needs including disease management and medication oversight in accordance with physician orders.

NURSING VISIT, LAB DRAW
In-home blood draws- per physicians orders. Nurses will draw blood and deliver to lab.
REHABILITATIVE THERAPY SERVICES
- Physical Therapists can help a client improve mobility, balance and strength.
- Occupational Therapists work to help a client enhance upper body function and strength and improve skills needed to perform activities of daily living.
- Speech Therapists work to improve hearing and communication and receive assistance with memory and swallowing concerns.
SOCIAL SERVICES
Social Workers assist with personal and financial situations, long-term care planning, help coordinate community services and provide counseling and support.
HOME SAFETY EVALUATION
A rehabilitation therapist will conduct a thorough evaluation in your home, complete with an individualized report and recommendations, to help ensure you remain safe at home.
MEDICATION SET-UP AND REMINDER SERVICE
Registered Nurse provides initial medication assessment and sets up medications using a pill planner or pill box in accordance with the physician’s order. Home Health Aide is available to provide medication reminders, assure medication schedule is followed and pick up prescriptions.
Services are non-sectarian and open to all members of the community.
