Montefiore has been a leader in physical therapy and rehab care for over 100 years. And, with a $10 million renovation including a 67-bed rehab unit and expansion to our Beachwood campus, we are at the forefront of rehabilitation therapies and skilled nursing.
Our state-of-the-art 6,000 square foot Mandel Rehabilitation Pavilion encompasses innovative modalities for physical, occupational and speech therapy for post-hospital care patients and for outpatient therapy.
Our skilled nursing team and rehabilitation specialists, along with the physician, social worker and dietician work together to provide the best rehab program for each patient. Our therapists are caring, our environment is friendly and every patient has a private room. Our team provides one-on-one therapy to maximize independence and individual treatment plans. All treatment plans are outcome-focused. Patients benefit from regular interaction with our physicians, nurse practitioner team and staff of professionals.
We provide therapeutic care for a wide variety of diagnoses and needs. We offer physical, occupational and speech therapy for post-hospital residents, long-term care residents and those who require outpatient therapy or home care. It is our goal that each patient we serve reaches their highest level of function for a better quality of life.

Rehab Road™ is Jewel of Montefiore Expansion Project
Envision a busy street lined with a bank, restaurant, grocery store and movie theater. As a woman buckles herself into a Ford Taurus, a man uses an ATM and another sinks a putt on a nearby putting green. Does this sound like a typical day on a street near you? Guess again. It’s Rehab Road™, the crown jewel of the pavilion. Rehab Road™ enables therapy in a setting that poses realistic challenges for patients – from a grocery store, deli and bank, to stairs, curbs and ramps. It also features an ATM, kitchen, putting green and more. In our movie theater, patients encounter the dim lighting, floor incline and fold-up seats typical of most movie theaters. All this is part of a patient’s therapy, so they can live life more fully with less reliance on family and friends.
Rehab Road™ allows you to focus on activities of daily living (ADLs) in a setting that is upbeat and inviting, and specifically designed to prepare patients for community reintegration, return to their homes or simply for greater independence.
This concept of community reintegration places Montefiore a step above other rehab providers. It gives patients the chance to relearn daily activities in an environment that more closely resembles what they encounter when they leave rehab.
This is a place where we teach patients to navigate – some with wheelchairs, walkers, canes, or unaided – through heavy glass doors, turnstiles, and even up and down curbs and on bus steps, which are higher than standard steps.
Advanced Equipment for Diagnostics and Treatment 
Advanced, specialized equipment used for diagnostics and treatment, side-by-side with traditional weights and therapy treatments distinguishes Rehab Road’s programs from many others. Our high-tech Balance Master™ provides those with mobility issues a way to improve their balance. It is useful for those with orthopedic, neurological or with musculoskeletal issues. Our Smart Balance Master™ provides all the therapeutic advantages of the Balance Master, along with the high-tech posturography and assessment. They measure the vertical forces exerted by a patient’s feet to provide assessment and retraining in the voluntary motor control and sensory aspects of balance. It enhances balance, known as vestibular rehabilitation, and is useful for everything from vertigo and head injury to Meniere’s disease.
The EVA Walker supports the patient’s weight while walking, helping them to transition out of wheelchairs. The Pressure Mapping machine is available for inpatient treatment to assess pressure points of those confined to a wheelchair or bed.
After Surgery, Injury or Illness
Montefiore provides physical, occupational and speech therapy, as well as professional nursing care and medical monitoring.
Our round-the-clock skilled care unit, all with private rooms and bathrooms, is particularly beneficial for patients who require orthopedic rehabilitation, are recovering from specific injuries or surgeries, or have suffered congestive heart failure, vascular disease or stroke.
A comprehensive discharge plan is prepared to help ensure a residents’s smooth transition home. Some choose to enjoy a transition or respite stay at The Weils (our assisted living facility in Chagrin Falls) with the support of Montefiore at Home, our home healthcare program. In addition, for orthopedic patients, electronic updates are available to family members and bi-weekly updates are sent to physicians.

We offer post-hospital care for all facets of disease and injury from fractures and stroke to speech disorders. Our Post-Hospital Care Center also includes two specialty orthopedic annexes, as well as a heart failure unit created to educate and advise heart patients and their families about cardiovascular disease.
Montefiore also has therapists certified in lymphedema management and a variety of other specialties including orthopedics, cardiac rehab, Myofascial Release, vestibular rehab and senior strength training for both inpatient and outpatient clients. For patients who are recovering but need some extra assistance prior to being discharged to home, we have efficiency suites at Montefi
ore available for short-term, transitional stays. Under the supervision of nursing and therapy, patients staying in the transitional suite
will be responsible for managing their medications and other activities that are part of daily life such as bathing, grooming, dressing, etc. The suite provides several amenities including a private bathroom with shower, dresser, dishwasher, microwave and refrigerator to help patients ease into the transition from rehab to life at home. This service is covered under Medicare or private insurance.
Life-Enhancing Amenities
Montefiore offers several amenities adding to the quality of life and enriching the environment for our clients. Once a month events are held for all the rehabilitation pavilion’s patients. Often at these holiday-themed gatherings, the festivities include activities that work with the patient’s rehab therapy. In the past these have included hitting a piñata and pie baking. Additionally, our Breakfast Group gets together weekly to make their own food and practice this important activity of daily living.
