From skilled nursing to physical, occupational and speech therapies, Montefiore’s homecare professionals offer the best home healthcare – wherever you live! Montefiore delivers compassionate, individualized healthcare right

Whether you are an early bird or a night owl, Montefiore homecare specialists will work with your schedule to meet your healthcare needs. Our flexible scheduling allows you to manage your day in a healthy way.

Our homecare nurses and therapists act as case managers, coordinating with the physician and medical team to implement an individualized care plan for each patient.

These services are covered by Medicare, Medicaid/Passport and most insurance companies:

• Nurses and therapists with expertise in orthopedics, cardiac and respiratory care, geriatrics, oncology/palliative care, pain management and low vision rehabilitation.
• Expert care for those with diabetes or who require IV therapy or wound care
• Home health aides to help with activities of daily living (such as bathing, grooming
  or dressing)
• Russian interpreter
• On-call availability 24-hours a day

Our homecare team serves clients in Cuyahoga, Lake and Geauga counties, as well as parts of Summit and Portage counties. Montefiore homecare is non-sectarian and open to the community.

Home Health Aide and Personal Care
Assistance with personal care such as bathing, dressing, grooming, meal preparation, incontinence care and transfers.

Sitter Care, Respite and Compassion Care
Available to family members and caregivers who may need relief from the ongoing care of their elderly loved one.

Chore Services and Homemaking
This service is designed to restore or maintain the client’s healthy living environment by performing a variety of housekeeping tasks and services.

Respite and Companionship Care
Available to family members and caregivers who may need relief from the ongoing care of their elderly loved one.

Montefiore provides a full spectrum of healthcare services including:

• Registered Nurse and Case Management Services
• Registered Nurses (RN) provide support and serve as the liaison with the client’s physician to coordinate healthcare needs, RN case management, 24-hour emergency telephone access, and communication to primary caregiver and referrals to other services as needed
• Licensed Practical Nurse (LPN) services are under the supervision of an RN; the LPN provides nursing care or treatments
• Physical, Occupational and Speech Therapy
• Physical therapy: balance, strength, coordination, structural barriers and hazards in the home, and gait and transfer functional mobility
• Speech therapy: Difficulties with swallowing (dysphasia), speech precision, aphasia, and cognitive deficits
• Occupational therapy: Home safety, Activities of Daily Living (ADLs), fine motor tasks, and low vision needs including home modification and magnification devices

Medical Social Services
Social workers assist patients and families with long-term planning and obtaining a living will or power of attorney. They help coordinate community services and provide counseling and support.

Personal Care and Homemaking Services
Assisstance with personal care such as bathing, dressing, grooming, meal preparation, incontinence care and transfers. Home health aides also perform a variety of housekeeping tasks and services to help restore or maintain the client’s healthy living environment.

Wellness Services

Bone Density Screen*
A non-invasive ultrasound screening service to determine fracture risk.

*Bone density equipment made possible by the Wolf Family Foundation, a supporting foundation of the Jewish Community Federation of Cleveland.

We have now partnered with Philips Lifeline Medical Alert Service and offer monthly wheelchair clinics on the first Tuesday of every month with Health Aid of Ohio. We also provide transportation services if you need assistance with running errands or getting to medical appointments. Transportation services are performed only with the patient’s own vehicle.
