Medicaid Liasison

Montefiore’s newly renovated facility in Beachwood, Ohio, offers state-of-the-art post-hospital care, homecare, dementia care, long-term nursing home care, assisted living and hospice services.

Montefiore’s Finance department has an opening for a Medicaid Liaison.  This individual will work with residents, families, attorneys, county and state agencies to assist residents secure Medicaid benefits by facilitating the application process and securing necessary documentation.

The qualified candidate will have a Bachelor’s Degree or equivalent, previous experience with Medicaid assistance rules, knowledge of community resources, excellent organizational, interpersonal and written communication skills, a high level of attention to detail and proficiency in MS office products.  Successful candidate will have the demonstrated ability to work with limited supervision and the ability to coordinate multiple assignments and prioritize workload.  Prior experience working in a long-term care or hospital environment is preferred.

Hospice RN Case Manager

Montefiore is dedicated to a standard of excellence and personalized care, in order that people may continue to live their lives with dignity and security and as independently as they are able.

The Vinney Hospice of Montefiore has an opening for a Hospice RN Case Manager.  Candidate must be a licensed RN in the state of Ohio.  Hospice, Home Care and recent med/surg experience required.  Must have a valid driver’s license and reliable transportation along with a clean driving record.  Demonstrated organizational and time management skills.  Hours are 24 hours Monday through Friday with some on-call responsibilities.

For private duty home care and prn skilled visits available to service clients on the east side of Cleveland. Qualified individual must be compassionate, work well with seniors, be able to function independently, have own transportation, with a valid Ohio driver’s license and a clean driving record. At least 1 year of experience working in Home Care is preferred.

Work as needed for 4, 8, or 12 hours shifts – we need help days, evenings and nights.

Job Openings – Hospice

Montefiore is dedicated to a standard of excellence and personalized care, in order that people may continue to live their lives with dignity and security and as independently as they are able.

Our Hospice Department has the following openings available:

RN or LPN

Position requires availability for work on Friday, Saturday and Sunday nights, between the hours of 5 p.m. and 8:30 a.m. Must be a RN or LPN licensed in The State of Ohio. RN with a minimum 2 years, LPN 5 years, of nursing experience and recent medical/surgical experience. Home care, hospice, or oncology experience preferred. Must have a valid driver’s license, own transportation and a clean driving record.

STNAs

2 part-time openings:

Schedule Tues., Wed., and Thurs. 7 a.m. – 3 p.m. or 16 hours per week (may increase). Requires hospice and LTC experience. Home care experience preferred. Must have reliable car and good driving record. Travel on the east side, west side possible.
