Montefiore has been a leader in physical therapy and rehab care for more than 100 years.
And, with a $10 million renovation including a 66-bed rehab unit and expansion to our Beachwood campus, we are at the forefront of rehabilitation therapies and skilled nursing.
Our state-of-the-art 6,000-square-foot Mandel Rehabilitation Pavilion encompasses innovative modalities for physical, occupational and speech therapies for post-hospital care patients and for outpatient therapy.
A skilled nursing team and rehabilitation specialists, along with the physician, social worker and dietitian work together to provide the best rehab program for each patient. Our therapists are caring, our environment is friendly and every patient has a private room. Our team provides one-on-one therapy to maximize independence and individual treatment plans. All treatment plans are outcome-focused. Patients benefit from regular interaction with our physicians, nurse practitioner team and staff of professionals.
We provide therapeutic care for a wide variety of diagnoses and needs. We offer physical, occupational and speech therapy for post-hospital patients, long-term care residents and those who require outpatient therapy or home care. It is our goal that each client we serve reaches their highest level of function for a better quality of life.
Rehab Road™ is Jewel of Montefiore Expansion Project
Envision a busy street lined with a bank, restaurant, grocery store and movie theater. As a woman buckles herself into a Ford Taurus, a man uses an ATM and another sinks a putt on a nearby putting green. Does this sound like a typical day on a street near you? Guess again. It’s Rehab Road™, the crown jewel of the pavilion. Rehab Road™ enables therapy in a setting that poses realistic challenges for patients – from a grocery store, deli and bank, to stairs, curbs and ramps. It also features an ATM, kitchen, putting green and more. In our movie theater, patients encounter the dim lighting, floor incline and fold-up seats typical of most movie theaters. All this is part of a patient’s therapy, so they can live life more fully with less reliance on family and friends.
Rehab Road™ allows you to focus on activities of daily living (ADLs) in a setting that is upbeat and inviting, and specifically designed to prepare patients for community reintegration, return to their homes or simply for greater independence.
This concept of community reintegration places Montefiore a step above other rehab providers. It gives patients the chance to relearn daily activities in an environment that more closely resembles what they encounter when they leave rehab.
This is a place where we teach patients to navigate – some with wheelchairs, walkers, canes, or unaided – through heavy glass doors, turnstiles, and even up and down curbs and on bus steps, which are higher than standard steps.
Advanced Equipment for Diagnostics and Treatment
Advanced, specialized equipment used for diagnostics and treatment, side-by-side with traditional weights and therapy treatments distinguishes Rehab Road’s programs from many others. Our high-tech Balance Master™ provides those with mobility issues a way to improve their balance. It is useful for those with orthopedic, neurological or with musculoskeletal issues. Our Smart Balance Master™ provides all the therapeutic advantages of the Balance Master, along with the high-tech posturography and assessment. They measure the vertical forces exerted by a patient’s feet to provide assessment and retraining in the voluntary motor control and sensory aspects of balance. It enhances balance, known as vestibular rehabilitation, and is useful for everything from vertigo and head injury to Meniere’s disease.
The EVA Walker supports the patient’s weight while walking, helping them to transition out of wheelchairs. The Pressure Mapping machine is available for inpatient treatment to assess pressure points of those confined to a wheelchair or bed.
Care After Surgery, Injury or Illness
Our round-the-clock skilled care unit, all with private rooms and bathrooms, is particularly beneficial for patients who require orthopedic rehabilitation, are recovering from specific injuries or surgeries, or have suffered congestive heart failure, vascular disease or stroke.
A comprehensive discharge plan is prepared to help ensure a patient’s smooth transition home. Some choose to enjoy a transition or respite stay at The Weils (our assisted living campus in Chagrin Falls) with the support of Montefiore at Home, our home healthcare program. In addition, for orthopedic patients, electronic updates are available to family members and bi-weekly updates are sent to physicians.
We offer post-hospital care for all facets of disease and injury from fractures and stroke to speech disorders.

For 131 years, Montefiore has cared for Cleveland’s aging Jewish and general community. We continue to provide excellent and comprehensive care to individuals as they age and support to families. We offer a full continuum of care including post-hospital, rehabilitation therapies, skilled nursing, homecare, private duty services, Alzheimer’s/dementia care, long-term nursing home nursing home care, assisted living, hospice and palliative services and outpatient therapy.
In 2007, Montefiore celebrated its 125th anniversary and completed a three year expansion and renovation project. This included the refurbishment of all resident rooms, new atrium, offices, Shabbat elevator, gardens, courtyard, children’s play areas, private family dining room, kitchen and wireless connections. The true jewel of the project is The Mandel Rehabilitation Pavilion and 67 bed Post-Hospital Care Center featuring Rehab Road™, an indoor simulated neighborhood to help relearn the activities of daily living.
In 2010, a specialty orthopedic annex was created. Due to its tremendous success, a second orthopedic annex was added in 2011. Also in 2011, a heart failure unit was completed to educate and advise heart patients and their families about cardiovascular disease. We also expanded our services with an at-home sleep apnea testing and therapy program, partnered with Philips Lifeline Medical Alert Service and offer monthly wheelchair clinics with Health Aid of Ohio. Montefiore also has therapists certified in lymphedema management and a variety of other specialties including orthopedics, cardiac rehab, Myofascial Release, vestibular rehab and senior strength training for both inpatient and outpatient clients. In early 2012, a transitional care suite for rehab patients was opened. We also provide transportation services if you need assistance with running errands or getting to medical appointments. Transportation services are performed only with the patient’s own vehicle.
Transitional Care Suite
For patients who are recovering but need some extra assistance prior to being discharged to home, we have a transitional suite at Montefiore available for short-term, transitional stays. Under the supervision of nursing and therapy, patients staying in the transitional suite will be responsible for managing their medications and other activities that are part of daily life such as bathing, grooming, dressing, etc. The suite provides several amenities including a private bathroom with shower, dishwasher, refrigerator and desk to help patients ease into the transition from rehab to life at home. This service is covered under Medicare or private insurance.
The Stella Kleinman Rehabilitation Therapy Gym
An additional 650 square feet of space dedicated to rehab therapy treatments – tripling the size of the previous space. It is a “specialty gym” designed to promote, restore and maintain overall fitness and health, while presenting an innovative approach to physical wellness and rehab therapies.
This airy and spacious rehab therapy gym is full of natural light and is bathed in soft colors, emanating a warm, soothing and healing environment. The roomy expansion has state-of-the-art modalities and therapeutic equipment along with a kitchenette and washer/dryer to provide patients the tools to help them get stronger, more independent and ready to go home.
Relief Services for Lymphedema Sufferers: Montefiore has extensive expertise to help Lymphedema sufferers combat, manage and control symptoms that have occurred from Lymphedema. According to Adrianne Shimek, director of Rehabilitation Services, “We use a combination of manual compression lymphatic massage implemented by our certified staff of Lymphedema therapists – which is available for both inpatient and outpatient clients.”
Signs of Lymphedema include: swelling of either part of the whole arm or leg; swelling of the head or neck; feeling as if an arm or a leg is too heavy or feels tight; range of motion is restricted; discomfort in the affected limb(s); and recurring skin infections.
“We can further provide elastic sleeves or stockings to be worn throughout the day, bandaging limbs overnight to reduce fluid and increase function, plus introduce remedial exercises during one’s daily routine,” continues Shimek.
Montefiore also has therapists certified in Myofascial Release and a variety of other specialties, including: orthopedics, cardiac rehab, vestibular rehab and senior strength training for both inpatient and outpatient clients.

Montefiore offers several amenities adding to the quality of life and enriching the environment for our clients. Once a month events are held for all the rehabilitation pavilion’s patients. Often at these holiday-themed gatherings, the festivities include activities that work with the patient’s rehab therapy. In the past these have included hitting a piñata and pie baking. Additionally, our Breakfast Group gets together weekly to make their own food and practice this important activity of daily living.
Posh Nosh Café
The Posh Nosh Café is a unique dining experience at Montefiore. We offer a wide menu selection that will spark excitement in your taste buds. We start our mornings off with several healthy breakfast choices to choose from: omelets, wraps and on the lighter side, bagels and toast. Our newest menu item is the chipotle veggie burger or wrap. We offer two healthy soups daily, salads and a variety of 14 different sandwiches and wraps.
Our summer hours are: Monday through Friday, 9 am to 3:30 pm. Saturdays from 11 am to 4 pm and Sundays from 11 am to 2 pm. Whether you are taking out or eating in, we hope you’ll stop by and join us for breakfast or lunch. Call 216.910.2480.
MoMart Gift Shop
Located in the lobby, the MoMart Gift Shop has a number of moderately prices gift items, clothing, jewelry, toys, purses and accessories. Run and managed by the Montefiore Auxiliary, volunteers are courteous and encourage browsing by residents and family members. Gift-wrapping is available.

Beauty and Barber Salon
Montefiore’s full-service beauty and barber salon is located on the first floor near the elevators. The salon offers haircuts, shampoos and sets, blow-drying, perms, highlighting and color, waxing and manicures.
Please call 216.910.2478 to set up an appointment or ask your social worker or nurse if you need assistance. Salon services must be paid for with cash, check or your resident trust account at the time of service. Price lists are available upon request. Open Tuesday to Friday from 10 am – 4 pm.*

E-mail loved ones who don’t have a computer! Thanks to a generous donor, Montefiore is proud to introduce “Presto” – a combination of the Presto Printing Mailbox and Presto Mail Service. Learn about this brand new way to stay in touch with your loved ones at Montefiore without their need of a computer.
What Presto does:
- It allows you to send an e-mail to loved ones who don’t use a computer or the internet;
- It transforms e-mails into beautiful, easy-to-read e-letters with photos and attachments.
How it works:
- Send an e-mail, photos and other documents to a Presto-provided
- The Presto Mail Service transforms e-mailed messages and photos into printable, full color “e-letters”;
- The Presto Printing Mailbox automatically retrieves messages via the phone line and prints them out.
The service provides residents a decorative, hand-delivered message sent from family and friends using our custom e-mail address once you sign up. E-mail firstname.lastname@example.org to register or call 216.910.2741 for more information. After registering, family and friends will receive “how to” instructions and can begin sending unlimited messages to their loved ones via e-mail. The Presto Mailbox converts the message into a beautiful letter with a font size large enough for all to read.
Our dedicated volunteers will hand deliver the message to residents within 24 hours of receiving it. It’s that easy.
“Memories of the Sea”
Our custom-designed art glass water sculpture, located in the atrium, reminds us that art and technology together can bring us much joy and pleasure. “Memories of the Sea” uses the age-old technology of glass blowing to bring memories of underwater adventure to life. It is approximately 10 feet wide and 8 feet high and consists of stainless steel, glass and copper elements.
This 17,000 pound, eight-foot-tall underwater ecosystem is located in the heart of Montefiore’s newly renovated atrium. The aquarium consists of several colorful types of saltwater fish including clown and trigger fish. This state-of-the-art ecosystem is a beauty for all to enjoy. Visitors of all ages will take pleasure in watching the extraordinary sea life and the uniquely life-like colorful coral reefs.
Entrance Gardens, Outdoor Gardens and Courtyard
Our beautifully landscaped gardens offer residents, visitors, families and friends wonderful encounters with colorful foliage, sculptures and a stunning granite fountain. The season-less courtyard are handicapped accessible and serve as an outdoor retreat complete with dining patio, umbrella tables and chairs to encourage socialization and intergenerational activity between residents and children, who will enjoy the wooden climbing gym and slide. Shade trees and paved pathways for strolling, lush flower beds, bird baths and bird feeders all make our gardens an inviting and wonderful place to get together.
