Assistance with personal care such as bathing, dressing, grooming, skin care, meal preparation, bathroom assistance, incontinence care and transfers.

Relief for caregivers in the home or other healthcare setting. We’ll keep your loved one company so you can take a break.

Homemaking Services
Housekeeping tasks to maintain a safe and clean living environment. We can also run errands.

PHILLIPS LIFELINE MEDICAL ALERT SERVICE

Montefiore at Home will help you maintain your independence and feel sage in the comfort of your own home. We have partnered with Phillips Lifeline to provide prompt caring assistance at the push of a  button, 24 hours a day, 365 days a year. Subscribers will receive a pendant (necklace or bracelet) that can be used to contact Lifeline response associates in case of an emergency. Order Lifeline service today and feel safer tomorrow. TRANSPORTATION SERVICES

Our Private Care staff can provide transportation in conjunction with your personal care needs. Transportation is available for medical appointments and other outings as requested. We will accompany the client and use the client’s car.

Registered Nurses (RN) provide support and serve as the liaison with the client’s physician to coordinate healthcare needs including disease management and medication oversight in accordance with physical orders.

Registered Nurse Case Management Services

Registered Nurses (RN) provide support and work closely with the client’s physician to provide for and coordinate care needs.

Registered Nurse Home Visit

Cost on a per visit basis.
Licensed Practical Nurse (LPN) – Under the supervision of a RN, the LPN provides nursing care and/or treatments.

LPN Services

Under the supervision of an RN the Licensed Practical Nurse Services will provide nursing care or treatments.

REHABILITATION THERAPY SERVICES

Physical Therapists can help a client improve mobility, balance and strength.
Occupational Therapists work to help a client enhance upper body function and strength
Speech Therapists work to facilitate communication and swallowing concerns.

Therapists will assist clients who desire additional rehabilitative services that are not covered by the insurance company.

MEDICAL SOCIAL SERVICES

Social Workers assist with long-term care planning, help coordinate community services and provide counseling and support.

HOME SAFETY EVALUATION

A therapist will conduct a thorough evaluation in your home, complete with an individualized report and recommendations, to help ensure you remain safe at home.

MEDICATION SET-UP AND REMINDER SERVICE

Registered Nurse provides initial medication assessment and sets up medications using a pill planner or pill box in accordance with the physicians’ order. Home Health Aide available to provide medication reminders, assure medication schedule is followed and pick up prescriptions.

IN-HOME MEMORY CARE PROGRAM

Montefiore’s new In-Home Memory Care Program is designed to provide quality memory care services and support at home so individuals may live independently in their residence as long as possible.

Montefiore has assembled a kind, caring and compassionate group of memory care experts to help their clients receive needed support and care. Specially trained staff members recognize the individuals behind the diagnosis and their desire for a high quality of life.

Our team of experts offers a coordinated and comprehensive approach to those with memory impairment to ease the stress of decision-making on family, caregivers and loved ones. Together, our goal is to ensure the safety and overall well-being of a loved one.
