Registered Nurse Case Management Services

Registered Nurses (RN) provide support and work closely with the client's physician to provide for and coordinate care needs

RN telephone case management, weekly calls, 24-hour emergency telephone access, communication to primary caregiver and referrals to other services as needed other services as needed.

Registered Nurse Home Visit

Cost on a per visit basis

LPN Services

Under the supervision of an RN the Licensed Practical Nurse Services will provide nursing care or treatments.

Rehabilitative Therapy

• Physical Therapy
• Occupational Therapy
• Speech Therapy

Therapists will assist clients who desire additional rehabilitative services that are not covered by the insurance company.

Medication Set-Up and Reminder Service

Registered nurse provides initial medication assessment and sets up medications using a pill-planner or pill strip. Home Health Aide follows up with patient to make medication reminders and assure compliance with medication schedule as established by the physician.

Medical Social Services

Social workers assist patients and their families with counseling and support, long-term planning and obtaining a living will or power of attorney. They help coordinate other services in the community that the patient may need.
