Thank you for your interest in supporting The Montefiore Foundation and/or The Weils, our assisted living and skilled rehab community in Chagrin Falls. Your generous contribution may be tax deductible and can be designated to any fund (see drop down box.) Please call our development department at 216-910-2350 if you would like to make your contribution by phone or if you have questions regarding the online form. You will receive an online acknowledgement upon receipt of your gift. We accept MC, Visa and Discover. Montefiore will send you a gift confirmation letter for tax purposes.

Annual Sustaining Campaign
Contributors to the Annual Sustaining Campaign support vital operating expenses, with 100% of your gift helping to bridge the gap between reimbursements and the true cost of care.
Each year, the generosity of our donors enables Montefiore to provide medical care and equipment, specialized therapies, religious programming, and engaging activities, as well as recognize the dedication of our exceptional staff and volunteers.
Levels of Donation
- Sir Moses Montefiore Society $10,000+
- Founder $5,000 to $9,999
- Benefactor $2,500 to $4,999
- President's Club $1,000 to $2,499
- Sustainer $500 to $999
- Century Club $100 to $499
- Friend $50 to $99
- Other
**President's Club gifts and greater may be paid in quarterly installments.
**Donors of $1,000 or more are invited to the Annual Donor Recognition Brunch.
