We
provide in-home compassionate care to support healthy,
independent living.
Using
the case management model, our home care nurses act
as case managers, coordinating with the physician and
medical team to implement an individualized care plan
for each patient.
Our
nurses specially trained in geriatric care provide skilled
observation, assessment and
treatment in the following areas:

Therapy/IV’s

with other referral sources in the community

24 hours a day.
Our
nurses keep in close communication with family members
in and out of town. We have Russian speaking nurses
on staff as well as a
full time Russian interpreter.

Montefiore Home Care offers Physical, Occupational,
and Speech Therapy visits in
the home both on a home care and an outpatient basis.
All of the Home Care therapists are licensed in Ohio,
and provide fluid communication with physicians, families,
and other health professionals.

Montefiore
is a PASSPORT Provider
PASSPORT
is a Medicaid waiver program that provides an alternative
to costly nursing home care. Eligible seniors can get
the help they need to live independently in their own
homes. PASSPORT provides in-home alternatives to nursing
home care for low income seniors.
