The geriatric assessment team is comprised of specially trained nurse, social worker and therapists who in conjunction with the patient's primary physician evaluate the specific needs of the senior exhibiting conditions including but not limited to memory loss, confusion, frequent falls, repeated hospitalizations, eating disorders and depression. Recommendations focus upon enhancement of at home function, risk reduction (eg. Falls, Polupharmacy), facilitation of health care delivery and socialization issues.
