HomeCare/Homemaking Services

Home Health Aide & Personal Care
Assistance with personal care such as bathing, dressing, meal preparation, incontinence care, transfers, etc.

Respite/Companionship Care
Available to family members/ caregivers who may need relief from the ongoing care of the elderly.

Chore Services/Homemaking
This service is designed to restore or maintain the client’s healthy living environment by performing a
variety of housekeeping tasks and services.

Independent Living Assistance
This service assists individuals in managing their household and handling personal affairs so that the
client can realize his/her maximum independence.

Wellness Services

Clinic/“POD” Services
Wellness clinic services/ “pods” of services
(2 hour time frame)

Menu of “pod” services as
follows:

Nursing screens:

  • Vital signs
  • Blood sugar
  • Ask a nurse
  • Hearing assessment and incontinence screening
  • Cholesterol screening

Integrative therapies – 15 minute treatment sessions – massage, reiki, reflexology
Balance, hearing and low vision

Health and Wellness Education
– senior health related areas

Massage Services
Includes therapeutic massages for treatment of specific diagnosed health conditions, relaxation
massages for stress relief and chair massages for the head, neck, shoulder and lower back.

Reiki Therapy
Touch therapy that uses energy through the laying on of hands to promote relaxation, stress relief
and comfort care.

Therapeutic Touch
An energy healing system that does not require touch to promote relaxation, stress relief and comfort care.

Foot and Hand Reflexology
An ancient healing method of applying pressure to the appropriate points of the foot or hand to alleviate stress and bring about balance in the body.

Professional Services
Registered Nurse
Case Management Services

Registered Nurses (RN) provide support and work closely with the client’s physician to provide for/
coordinate care needs. RN telephone case management, weekly calls, 24-hour emergency telephone access, communication to primary
caregiver, referrals to other services as needed.

Registered Nurse Home Visit
Cost on a per visit basis

LPN Services
Under the supervision of an RN the Licensed Practical Nurse Services will provide nursing care or
treatments.

Physical Therapy Occupational Therapy
Speech Therapy

Therapists will assist clients who desire additional rehabilitative services that are not covered by the
insurance company.

Home safety evaluation including completion of the 5-page written safety evaluation tool, communication to primary care giver and/or senior
living housing coordinator. Group exercise classes provided by an Occupational or Physical Therapist

Medication Set-Up and
Reminder Service

Registered nurse provides initial medication assessment and sets up medications using a pill-planner or pill strip. Home Health Aide follows
up with patient to make medication reminders and assure compliance with medication schedule as
established by the physician.

Medical Social Services
Social workers assist patients and their families with long term planning and obtaining a living will or power of attorney. They help coordinate
community services and provide counseling and support.
