Registered Nurse
Case Management Services
Registered Nurses (RN) provide support and work closely
with the clients physician to provide for/
coordinate care needs. RN telephone case management,
weekly calls, 24-hour emergency telephone access, communication
to primary
caregiver, referrals to other services as needed.

Nurse Home Visit
Cost on a per visit basis
LPN
Services
Under the supervision of an RN the Licensed Practical
Nurse Services will provide nursing care or
treatments.
Physical
Therapy Occupational Therapy
Speech Therapy
Therapists will assist clients who desire additional
rehabilitative services that are not covered by the
insurance company.
Home
safety evaluation including completion of the 5-page
written safety evaluation tool, communication to primary
care giver and/or senior
living housing coordinator. Group exercise
classes provided by an Occupational or Physical Therapist
Medication
Set-Up and
Reminder Service
Registered nurse provides initial medication assessment
and sets up medications using a pill-planner or pill
strip. Home Health Aide follows
up with patient to make medication reminders and assure
compliance with medication schedule as
established by the physician.
Medical Social Services
Social workers assist patients and their families with
long term planning and obtaining a living will or power
of attorney. They help coordinate
community services and provide counseling and support.
