MDS Nurse

This individual must conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and the policies and goals of the facility. A MDS Nurse is delegated the administrative authority, responsibility, and accountability necessary for carrying out assigned duties.

RNs preferred.

RN Weekend Nursing Supervisor

Weils is seeking an experienced RN to supervise and oversee campus (Assisted Living, Memory Care Unit, and Rehab Pavilion) operations on weekends.  Previous supervisory experience required.

1st shift/2nd shift, every Saturday and Sunday, 10-12 hours per day.

Activities Coordinators

The Weils is seeking a caring, energetic, dynamic and creative individual to enrich the lives of our residents.  The Activity Coordinator will assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan.

The ideal candidate will have previous experience working with the elderly and individuals with dementia, knowledge of MDS and care plan. AAC certification preferred.

Part Time, 24 hours per week (some evening and weekend work may be required)

Previous dementia related activities experience and certification preferred.
