Maltz Hospice House and Palliative Care Program Manager

**Immediate Full Time Opportunity**

Exciting opportunity to join the leadership team of the newly designed, Maltz Hospice House and Palliative Care Pavilion as Program Manager. Responsible for the overall management of this inpatient program including planning, organizing and directing the patient care services for the Hospice program in accordance with current standards and regulations while maintaining optimal quality of care. The Program Manager will work collaboratively with the Hospice Director to promote excellence, expand the visibility of the program while monitoring and maintaining compliance with all regulatory, State, Federal government and other accrediting standards.  Experience working collaboratively with all levels of care providers within the organization is essential for achieving growth and success of the program.

Qualified candidates will possess:

  • Registered Nurse, BSN with 3+ years of Hospice supervisory/administrative experience
  • Strong organizational, interpersonal communication, management and leadership skills
  • Interest and experience in program development and marketing/community outreach
  • Excellent problem solving and analytical abilities
  • Seasoned professional comfortable balancing multiple priorities and shifting assignments
  • Recognized as a credible resource in Hospice and Palliative care
  • Ability to work independently and exercise sound judgment and decision making
  • Work collaboratively with patient care service staff and clinicians to establish a highly regarded program.

Specific responsibilities include, but are not limited to:

  1. Administration, program planning and development, provision and evaluation of all in-patient hospice and palliative care services
  2. Administer, develop and update all policies and procedures relating to Hospice and Palliative care
  3. Provide leadership and direction to all staff and assure that they are aware of and in compliance with all program policies and procedures
  4. Supervise patient care and ensure quality care of all inpatient unit patients and families
  5. Coordinate the delivery of all staff orientation, supervision, timely performance feedback and reviews as well as on-going training needs
  6. Recruit, select and retain qualified staffing to meet the needs of patients for quality hospice and palliative care services at all times
  7. Deliver direct patient care on the unit as needed
  8. Participates in on-call duties with the management team providing 24/7 administrative back-up for the hospice and palliative care clinical team
  9. Assist in marketing and community outreach to promote and develop the program

Are you a RN with experience and passion for Hospice Care?  If so, Montefiore is looking for you.  We have   Registered Nurse opportunities available for our new, state of the art, in- patient Hospice unit opening this summer!

Oversee and provide patient care on this 6 bed inpatient unit. Work with our interdisciplinary team providing Hospice care to patients and their families in our facility.  This person will have at least 1 year experience in Hospice nursing.

Shifts available: 7am-3pm / 3p -11pm / 11p -7am

RN Patient Care Coordinator, Post-Hospital Care Unit

RN Patient Care Coordinator – 2 pm to 10 pm

Join the Montefiore leadership team as one of the Patient Care Coordinator on our 66-bed post-hospital  care unit.  Serve as a resident and family advocate and a liaison between the Charge Nurse and Physician.  This person will report to the Nurse Manager. This is an excellent opportunity for a RN with a BSN preferred, with recent supervisory/management experience in acute care or LTC, must have strong nursing assessment and documentation skills, and be detailed oriented and able to work in a fast paced environment.  This person will have a current RN license in Ohio, have excellent assessment skills, knowledge of LTC regs. and MDS, must be detailed oriented, proactive, and able to work in a fast pace.  Supervisory experience is helpful.

Finance Billing Specialist


Responsibilities include the completion, review, transmittal, and collection of government, private insurance, and private pay long term, hospice, homecare and private duty claims. You MUST have previous (2-3 years) experience with health care third party billing. Prior experience working in a long-term care or hospital environment is preferred. Knowledge of community resources, excellent organizational, interpersonal and written communication skills, and a high level of attention to detail and proficiency in MS office products is required. You will also have the demonstrated ability to work with limited supervision and the ability to coordinate multiple assignments and prioritize workload.

    Position requires availability for work on Friday, Saturday and Sunday nights, between the hours of 5 p.m. and 8:30 a.m. Must be a RN or LPN licensed in The State of Ohio. RN with a minimum 2 years, LPN 5 years, of nursing experience and recent medical/surgical experience. Home care, hospice, or oncology experience preferred. Must have a valid driver’s license, own transportation and a clean driving record.

    Requires hospice and LTC experience. Home care experience preferred. Must have reliable car and good driving record. Travel on the east side, west side possible.

The Montefiore Foundation seeks a dynamic, outgoing, creative and engaging professional to serve as Development Associate focusing on successful implementation of strategic development priorities including cultivating a portfolio of donors to meet development goals. Montefiore is looking for an assistant for our Posh Nosh Café.  Responsibilities include food preparation, kitchen duties, and the assisting the Café Manager as needed. Hours will be Saturday and Sunday 10:30 a.m. – 5:30pm. **Immediate opportunity** The qualified candidate will have cooking knowledge, knowledge of kitchen sanitation guidelines, and the ability to work in an open kitchen.  Certification in ServSafe is required. Experience with food service in a healthcare facility is a plus, Good attendance record, and excellent organizational, customer service and interpersonal communication skills required.

Director of Nursing – The Weils is currently seeking an energetic, experienced RN that is committed to providing quality care in a person-centered environment. This person will be responsible for day to day planning, staffing, and direction of the overall clinical care provided at our 29-bed rehab pavilion. Qualified candidate will also oversee the management of our 75-suite assisted living campus, including 8-beds for specialized memory care and an expansion of an additional 18-bed memory care pavilion coming in spring of 2015. This is a full-time position with on-call responsibilities. Prior nurse management experience is essential. BSN; Master’s Degree preferred. Previous experience in a geriatric healthcare setting required.
