RN Patient Care Coordinator, Post-Hospital Care Unit
RN Patient Care Coordinator - 2 pm to 10 pm
Join the Montefiore leadership team as one of the Patient Care Coordinator on our 66-bed post-hospital care unit. Serve as a resident and family advocate and a liaison between the Charge Nurse and Physician. This person will report to the Nurse Manager. This is an excellent opportunity for a RN with a BSN preferred, with recent supervisory/management experience in acute care or LTC, must have strong nursing assessment and documentation skills, and be detailed oriented and able to work in a fast paced environment. This person will have a current RN license in Ohio, have excellent assessment skills, knowledge of LTC regs. and MDS, must be detailed oriented, proactive, and able to work in a fast pace. Supervisory experience is helpful.

Montefiore, A leading senior care provider is seeking a seasoned and dynamic Manager of Social Services to join its interdisciplinary team of healthcare professionals.
The ideal candidate will have demonstrated success in:
- Leading a team of social workers providing for the psycho-social needs of residents and families on a high volume post-hospital care rehab unit;
- Designing specialized programming interventions for behavior management, education and community outreach;
- Developing social service assessments, documentation, care plans and interventions to achieve goals and address resident needs;
- Working with residents, families and caregivers regarding advanced directives and resident rights;
- Providing support groups, outreach and educational programs;
- Planning for discharge and support services to ease resident transitions.
Applicants must possess a Masters degree in Social Work, current Social Work license, and a minimum of 5 years experience managing social services in a hospital or post-acute care facility. Ability to support residents and families through all aspects of their care and discharge planning. Must have CMS regulation knowledge, and strong documentation skills. Weekend rotation required. Alzheimer’s and Dementia special care as well as supervisory and teaching experience preferred.

Montefiore is currently seeking an experienced Nurse Practitioner to work as part of an interdisciplinary team to provide health care to patients in a variety of inpatient settings. Responsibilities include review of patient needs prior to and upon admission, management of both episodic and chronic medical needs, QA, infection control, assess medical necessity of therapies and services, provide clinical education and training of residents, caregivers and staff and conduct employee health screenings.
Masters Degree from an accredited Nurse Practitioner program, active license with Ohio Board Of Nursing, and certification as a Geriatric, Adult, or Family Nurse Practitioner is required. Must have strong knowledge of the common geriatric diseases and excellent verbal and written communication skills.

RN with a minimum 2 years, LPN 5 years, of nursing experience and recent medical/surgical experience. Home care, hospice, or oncology experience preferred. Must have a valid driver’s license, own transportation and a clean driving record.
