Scope
of Service
Program
Summary:
The Access at Home Wellness Program is a Mt. Sinai Health
Care Foundation grant supported program, which aims to improve
the quality of life and general well being of seniors. The
community-based program has allowed Montefiore nursing facility
to provide a continuum of care, which reaches seniors within
the chronic care populations. The wellness program is a positive
addition to the greater Cleveland area's services for seniors
and a way to provide preventative care - to avert more serious
illnesses. All wellness services are free to its participants.
The wellness program offers nursing, home safety, audiology,
and integrative services, including reiki, massage, reflexology,
and aromatherapy.
Professional
Disciplines
and Services Provided:
Nursing - blood pressure, glucose, and cholesterol screening.
Health information/education in an "ask a nurse"
format. (RN and LPN).
Reiki
and Massage Therapist - 15 minute chair or table massage.
Reiki treatments, reflexology and aromatherapy services. Education
in all integrative therapies. Staff therapists work the reiki
volunteer program.
Home
Safety Evaluations - Occupational Therapists evaluate
senior's home environment and offer education in all areas
of safety management.
Audiology
- Audiology assessments monthly by a volunteer, licensed
audiologist.
Podiatrist
- Free podiatry screenings by a licensed doctor.
Education
- All Access at Home personnel are available to provide both
informal and formal education on any health care/wellness
topics. All professional staff participate in mentoring both
volunteer staff and students.
Services
Area and Target Audience:
Seniors seeking wellness services and the frail elderly within
Cuyahoga County.
Hours
of Services:
9:00 A.M. - 5:00 P.M. - Monday through Friday for routine
services, clinics. Health fairs, education program, mentoring
- on demand and during evening hours.
Link with Montefiore Home Health
and Hospice Program:
The Access at Home Program and its employees adhere to all
Montefiore at Home administrative policies and procedures.
Access at Home is a part of the Community Services Professional
Advisory Council and Program. Access at Home is managed by
the Vice President of Community Services. Access at Home staff
working for the program offer all of the wellness services
to the nursing home, home health, and hospice patients and
families.
